I hereby certify that all information set forth in this employment application are true and complete to the best of my knowledge. I understand that misrepresentation or omission of facts on this application or any other Giving Tree Child Development Center (GTCDC) records will be cause for the rejection of my application, my granting of an interview, or the immediate termination of my employment. I also understand that should I be employed by GTCDC, I have the right to terminate my employment at any time and that GTCDC may at its discretion terminate my employment with or without prior notice.

I hereby authorize GTCDC to investigate my background and all references given and authorize them to disclose to us. I release GTCDC and/or any person, organization or previous employer from any and all liability that may result from any investigation into my background conducted by GTCDC and its management staff.
